Reference forms

Before your application is complete and ready to be processed you must submit four (4) completed reference forms. Your application will not be processed until we receive all of your reference forms.

Follow the instructions below to obtain reference forms, and return them to the Collegiate Mobilization Team.

• Download Reference form (Adobe Acrobat format) and save it to your computer. Send an email to your references with the Reference form as an attachment.
• Distribute the reference forms to all four references to include the following people:
  a. Your Pastor or Church Staff
  b. Your Baptist Campus Minister (BSU/BCM) or Church Student Minister
  c. A Professor or Employer
  d. A friend
• Ask each reference to email the completed reference form as an attachment to StudentTeam@imb.org.
• If your references are unable to complete the reference forms electronically please call 800-999-3113 ext. 1546.
